- Remote Patient Monitoring Devices for Eligible Patients Eligible patients may be provided with remote patient monitoring devices— including a blood pressure monitor, blood glucose meter, weight scale, or pulse oximeter—through contracted healthcare providers, subject to insurance coverage and applicable qualification requirements.
- Clinically relevant health data collected from FDA-cleared home devices Vital sign data stored in the CyberMDCare App is shared instantly with the patient’s primary physician, enabling timely medical guidance.
- Enhanced Provider Communication Readings that fall outside physician-defined thresholds are routed to the care team for review and follow-up in accordance with clinical protocols.
